According to an article in the Oregonian, the Oregon Occupational Safety and Health Administration fined Stimson Lumber $5,000 for infractions related to a fatal accident in May.

Oregon's OSHA said the Portland-based forest products company didn't adequately train an employee who died from a hydraulic-equipment explosion.

The U.S. Department of Labor's Occupational Safety and Health Administration (OSHA) has cited a Home Depot store in Chicago for two repeat and one serious alleged safety violations, including failing to remove damaged equipment from service and train employees on safety procedures. The company faces proposed penalties of $55,000.

A Lowe’s employee was killed in a storage facility in Charlotte, N.C., right across the parking lot from one of the retailer’s stores, according to an article in the Charlotte Observer. The 35-year-old man was standing under a garage door when it suddenly fell and hit him on the head at 11 a.m. on June 4.
